About the program
Wellways Australia is the first point of contact for all Australian Government Carer Gateway funded services in South West Sydney and across Queensland, aiming to help carers access practical information and advice and connect with services in their local area. Carer supports and services will be delivered using a network of local Carer Gateway regional service providers Carers and those who provide a caring role to someone in their life who has a disability, mental health issue, chronic health condition, terminal illness or are frail aged.
About the role
The Intake and Registration Officer is a newly created role responsible for undertaking intake and registration services for carers accessing support provided by the Carer Gateway across Queensland.
Key responsibilities include, however, are not limited to:
Ensuring that carers meet the eligibility requirements for accessing services and supports
Registration of carers into the Wellways Client Management System
Providing information to carers on the process for intake needs assessment and planning, and available supports
Scheduling assessment and planning appointments for Carer Support Workers
Skills & Experience:
Demonstrated knowledge of caring roles and the impact on carers health and wellbeing to determine support requirements
Experience in coordinating responsive individualised support to individuals, families, young people and children with caring roles
Contact Centre experience is desirable
Evidence of working rights within Australia such as a passport or birth certificate
NDIS Workers Orientation Module
A current valid Driver's License
National Police Records Check
Working with Children Check (Current Blue and Yellow Card)
