Seniors In Networked Communities Program (SINC) Administrator
This is an administrational position, overseeing an entry level in home support service to individuals who are aged over 65 years under Commonwealth Home Support Program (CHSP).

CatholicCare is a leading provider of aged care services in the ACT and surrounds. CatholicCares Commonwealth Support Program is looking for an enthusiastic and committed worker, ideally with experience working in aged care and CHSP, to expand and grow the program.
CatholicCare is seeking to employ a SINC Administrator
This purpose of this role is to be the first point of contact for client enquiries for CHSP (Commonwealth Home Support Program). The SINC Administrator plays a key role in the delivery of high quality community based care.
The successful applicant will be responsible for contributing to the growth and development of the program and ensuring that our clients receive individualised, person centred, enablement focussed and responsive support.
The SINC Administrator will be required to work within a person centred, wellness and reablement or restorative framework as outlined in the CHSP Manual and in line with the Aged Care Quality Standards.
The role will work with consumers to identify needs and goals, develop support plans and monitor services.
Responsibilities include, but are not limited to: managing referrals and the client waitlist; on boarding clients to the program; liaising with the rostering team; liaising with clients and ensuring that services are consistent with My Aged Care support plans; and referring back to My Aged Care should individuals needs change.

The ideal person we are looking for has:
- Certificate in Aged Care or similar and relevant experience.
- High computer literacy including word processing and Excel spreadsheet.
- Good communication and interpersonal skills.
- High level of attention to details and a quick learner.
- Ability to juggle multiple tasks with effective time management skill and meet deadlines

You will also be required to undergo a National Police Check.
