Coast & Country Primary Care (CCPC) is an independent not for profit organisation, providing programs and services that support the provision of primary healthcare and community services to the Central Coast community. Based in Erina, we offer a range of programs and services across the Central Coast.
About the role

Key responsibilities will include:
Managing enquiries regarding NDIS services
Developing, implementing, and coordinating social activities for NDIS participants and Continuation of Support participants
Managing our NDIS administrative systems and processes including booking and referral systems and processes
Rostering & maintaining supports for participants
Providing on-call service for NDIS services, particularly Direct Supports
Managing Australian Health Professionals (AHP) rosters, conducting activities to increase AHP contractor levels, responding to AHP enquiries and coordinating service provider contracts
Coordinating Home and Community support for non CCPC participants

Relevant tertiary qualification, (e.g., Certificate III in Community Services, Health or similar) preferred.
Previous experience in a similar position, and a demonstrated passion for the disability, community servies or mental health sector.
Extensive experience within a high-level administration capacity, with advanced skills in MS Office
Working knowledge and demonstrated experience using database software
Proven ability to identify, prioritise and manage multiple tasks in an environment of competing demands
Knowledge of issues experienced by people with disabilities (inclusive of psycho-social disability), their families and carers. This should include people from Aboriginal and Torres Strait Islander (ATSI) and from culturally and linguistically diverse (CALD) backgrounds
Demonstrated experience working independently and as a team member
Demonstrated ability to communicate sensitively and effectively to build rapport with clients, service providers and other organisations
A valid Working with Children Check for paid employment to be obtained and maintained at your own expense
Eligibility to work in Australia on a permanent basis
Drivers license and access to a reliable and insured motor vehicle
