Aged Care / NDIS Coordinator (6 month fixed term)

Established in 1992, IWAA is a not for profit organisation working in the community sector providing various services like Aged Care, Settlement, Disability and Community Development to communities across SE Qld, NSW and Vic.

There is currently an opportunity  in our Condell Park Office. A brief summary of the role is:

Intake and Recruitment

  • Assistance with Referral of clients to Government Assessors (NDIS, Access Point and MAC)
  • Management and checking of approvals

Rostering

  • Roster Home Care Employees (HCEs) in accordance with care plans and service delivery requirements

Service Delivery

  • Provide comprehensive and holistic assessment of consumers needs and in conjunction with consumer, significant others and health professionals, assist with the design and development of responsive, detailed assistance plans based on the needs identified in the assessment, the consumers goals or subsequently from reviews.

Consumer Budgets/

  • Review consumer budgets in accordance to program

    Quality Assurance

  • Provide on-going monitoring of the quality of services received by clients, through regular client review and feedback, HCEs and significant others.

Staff Support

  • Coach/train HCEs on care tasks when required
  • Supervise and support HCEs

Other

  • Promotion and marketing of IWAA services
  • Liaise with service providers and suppliers

Essential Selection Criteria:

  1. A diploma in either Community Services, Human Services, Nursing or Aged Care and/or relevant experience.
  2. Demonstrated ability to work effectively with CALD and mainstream communities.
  3. Demonstrated knowledge and understanding of the special needs of frail aged and those with disabilities including their rights and responsibilities.
  4. Demonstrated understanding of working with some supervision in the context of a community based organisation.
  5. Demonstrated ability to manage multiple tasks concurrently, be flexible in dealing with changing priorities and able to manage competing demands
  6. Sound communication skills, English language skills and demonstrated ability to resolve day to day conflicts.
  7. Well-developed customer relations skills and a commitment to providing quality services, and implementing continuous improvement.

    Desirable Selection Criteria:

  1. Ability to communicate in Arabic
  2. Knowledge of Federal & State Aged Care Services
